Road Repair Request Form

Please complete this form if you are aware of a maintenance issue with one of our county roads. Issues may include pot holes, vegetation blocking view, road damage, debris in road, etc. Please note that the county is not responsible for private roads, state highways, or roads within city limits. Please do not use this form in case of emergency.
